Checklist
In order to file your Bankruptcy case, certain information must be provided to the Bankruptcy court.
You are required to provide the following complete, accurate, and truthful information with your Bankruptcy Petition.

You are required to provide the following complete, accurate, and truthful information with your Bankruptcy Petition.
All assets, regardless of their location and regardless of who is in possession of the asset MUST be completely, accurately, and truthfully disclosed in the documents filed to commence your case. The value of each asset MUST be stated in those documents. A reasonable inquiry, by you, MUST be made to establish such amounts.
All liabilities, including debts due to family members, friends, and business associates MUST be completely, accurately, and truthfully disclosed in the documents filed to commence your case. A reasonable inquiry, by you, MUST be made to establish such amounts.
All your current monthly income and living expenses MUST be stated completely, accurately and truthfully. A reasonable inquiry, by you, MUST be made to establish such amounts.
In addition to the above required information copies, (Copies only, keep your ORIGINALS for your records) of the following are required to be provided:
- The last three years tax returns,
- Your last six months pay stubs,
- All current bills and statements (credit cards, car loans, mortgage, and utility bills),
- All current medical bills,
- All collection agency notices or statements,
- All Internal Revenue Service (IRS), Franchise Tax Board (FTB), Employment Development Department
(EDD), and State Board of Equalization Notices,
All foreclosure notices, Repossession notices, Bank Levy Notices, and Wage Garnishment Notices,
A current credit report, (no older than 30 days, and should include complete detail such as complete account numbers, the complete addresses, and the telephone numbers of your creditors),
Certificate of Credit Counseling (the certificate can be obtained from www.cccsatl.org, www.a123cc.com, or any another Bankruptcy Court approved provider,
Certificate of Post Counseling (to be completed after filing of your petition with www.bankruptcydebtoreducation.com).
Information that you provide during your case may be audited and failure to provide any information may result in such sanctions as the dismissal of you case and a further bar from filing another case under any chapter of Bankruptcy. Monetary sanctions can be assessed and levied as well as criminal penalties.
